There are several items need for a student to be allowed to participate in WCA Athletics.

1.  BIRTH CERTIFICATE - A copy must be turned in to the Athletics' Office. 

• Only after the birth certificate copy and Physical Evaluation Form have been turned into the Athletics' Office can we report the student's information to the AHSAA.  

• After this has been done, the student must wait 5 days before being eligible to participate in competition.

2.  PHYSICAL EVALUATION FORM - This form is filled out in part by the parent and in part by a physician.  It has two pages.  Please print these pages on ONE sheet of paper - front and back

• Students may not attend practice or compete unless an updated Physical Evaluation Form is on file in the Athletics Office.  

• This form is good for 365 days.  Then a new Physical Evaluation Form must be obtained and turned into the Athletics Office.

3.  MEDICAL CONSENT FORM - This form is filled out by the parent. It has only one page.

4.  STAR Program Completion Certificate - This is an on line program that must be completed by the athlete.  Click HERE to see instructions for completion of this program.

5.  ATHLETIC/ACADEMIC POLICY - This form is to be read & signed by BOTH the athlete & a parent.    Click HERE to READ the policy then Click HERE to print this form.
